VirusScanNT causing max CPU & very slow email download

I have a similar setup but haven't noticed any performance impact (and I
usually receive very large files trhu email). Are you checking all files
or just program files? (Properties->Detection->What to Scan) Since it'll
be very unlikely that someone makes a macro virus for PMMail, I don't see
any reason to check the text flow comming from your SMTP connection. You
only need to check any attachment at runtime.
