Here is a message from the mailing list from a while back.

I tried reindexing all the subfolders to see if that would bring back
any "phantom" unread messages. That didn't work, so I took a closer
look at the FOLDER.INI file in the Inbox directory.

The file looked like this:

Inbox|1|0|3;2;6;4;8;5;7;9;0;1|0|1|0|6|

(Note: The character shown by "|" is really hex DE.)

Some experimentation with a dummy account showed that the last digit
("6") should be 0 when there is no unread mail in the Inbox and no
unread mail in any of the subfolders under Inbox, so I shut down
PMMail and used a text editor to change the 6 to a 0. When I
restarted PMMail, the icon was back to normal, and it has behaved
normally since.

Your mileage may vary and all that, but it worked for me. I'd
strongly recommend backing up your PMMail setup before you try
anything like this on your own system.
