Actually, it wound up being unrelated ... something in the outbox in a
secondary account was causing the failure during send. Unfortunately, I
determined this by deleting the offending entry, and now I can't
recreate the problem - so problem solved for now, but no useful
diagnostics available to fix it permanently.
