Okay, I think I figured it out.  I had set "Editor" in the account
preferences to use an external editor for reading, so it always
did.

I unchecked that and said to use the internal editor.  It seems in
that case it will use the internal editor, which I can then
override by pressing ALT-X from the internal editor.  Am I correct
in thinking it will then take the "external editor" value from the
preferences tab, even though it's not checked?
