The situation in X11 is made more complicated because you encounter so many
wild keyboards standards. Ctrl-Ins and Shift-Ins might not be representable.

"CUA" stands for "Common User Access". It is an extensive set of user-
interface guidelines written by IBM in 1987, updated in 1991. It covers
everything from menu items to layouts to dialogs to keystrokes to mouse
operations. OS/2 PM was strongly CUA. Windows 95 is mostly CUA.
Most applications seem to toss the rules out the window.
